What’s printed on the card

The front has the cardholder’s photo, name and ID number. The back has the heart condition, ICD and blood thinner answers you give us, with Heart Children’s contact details.

What happens next

  1. Fill in the form and add a photo. Nothing is saved until you send it.
  2. Heart Children check every card before it’s printed. If they need anything, we’ll email you a private link.
  3. We post the card within 4 working days of approval and email you when it’s on its way. The first batch of cards takes about 4 weeks.

What you’ll need

  • The cardholder’s name and date of birth
  • Their congenital heart condition, if it’s been diagnosed
  • A recent passport-style photo
  • An email address and a postal address in Ireland

Questions

Child card or adult card?

Under 16s get a child card and 18s and over get an adult card. At 16 or 17 you can choose either.

Does it cost anything?

No. The card and postage are free.

What if the card is lost or the condition changes?

Order a new card with the up-to-date details.

How long does it take?

Cards are posted within 4 working days of being approved. The first batch of cards takes about 4 weeks.
